Swing Bed Program

As we age, there may be a need for an extended stay at a hospital facility. Once a patient is in an acute setting and they are in need of skilled services and have qualifying needs that the hospital can focus on to get the patient back to a prior level of functioning, the patient then becomes qualified for skilled nursing services. A care plan is designed to offer continuous nursing care in a hospital setting with a goal of rehabilitating the patient to a similar quality of life prior to their onset of illness. Our Skilled Nursing Team will assure that every patient who qualifies for skilled services receives a consult for nutrition, physical therapy, activities of daily living and social service's needs. The skilled services team will work with the patient to gain strength to be able to manage their healthcare needs at home. Skilled Nursing Services in a hospital setting are available to any community member who would like to remain in a hospital setting and qualifies for skilled nursing services. Our teams of professionals are here for you to make your recovery the best is can be. Our dedicated staff is compassionate about your healthcare.

Services include: Wound dressings, IV administration, Ostomy Care, Catherization, injections, vital signs, tracheotomy care, strengthening, social services consultations, nutrition services and consultations.

Educating patients regarding their disease processes is key to speedy recovery and disease management processes. Our team of trained staff members will educate the patient on illnesses and diseases. Examples of patient training may include topics such as diabetes, hypertension, and wound care. Our staff has the knowledge and resources to assure that education is available for any type of illness or disease process a patient may be faced with during their course of treatment.

Licensed physicians, Registered Nurses, Social Workers, Dieticians, Licensed Vocational Nurses, Certified Nurse's Aides, Physical Therapist, and support staff make up our dedicated Skilled Nursing Team. We are here to make your skilled nursing stay a positive experience for each of our patients.
