Swing Bed Program

As we age, there may be a need for an extended stay at a hospital facility. Once a patient is in an acute setting and is in need of skilled services and has qualifying needs that the hospital can focus on to get the patient back to a prior level of functioning, the patient then becomes qualified for skilled nursing services via our Swing Bed Program.

About Our Swing Bed Program

A care plan is designed to offer continuous nursing care in a hospital setting, with the goal of rehabilitating the patient to a similar quality of life prior to their onset of illness. Our skilled nursing team will assure that every patient who qualifies for skilled services receives a consult for nutrition, physical therapy, activities of daily living, and social service needs.

Specifically, the skilled services team will work with the patient to gain strength to be able to manage their healthcare needs at home.

Skilled nursing services in a hospital setting are available to any community member who would like to remain in a hospital setting, and who also qualifies for skilled nursing services. Our teams of professionals are here for you to make your recovery the best it can be. Our dedicated staff is compassionate about your healthcare.

While in our care and part of the Swing Bed Program, your care team will include you, the hospitalist, skilled nursing staff, rehabilitation therapists, a social worker, an activity coordinator, and our dietary manager.
